No, only clients admitted to Critical Care Units should be entered on HESN
There are many fields on the Personal Information screen. However, the minimum required values are name, DOB, Gender, Personal Identifier, Nationality, Health Region Information, Telephone, Address, and Alternate or Additional ID which is the Medical Record Number
For Newborn admitted to NICU prior to birth registration and the issuing of Personal Identifier, use additional ID to record Medical Record Number or the Mother ID or Father ID. Once the Newborn has a Personal Identifier, the information should be updated on HESN
Once admitted to ICU, you need to create 2 encounters. The first one with encounter type “Admission to Hospital” and encounter reason “Admission to Hospital” to catch the date of admission to hospital. This is important to determine denominators. The second one with encounter type “Admission/ Transfer to Units” and encounter reason “name of unit”. This will provide the name of the unit and the date of admission to that unit
When an encounter is created, it is placed in context under the client context. Any changes on the Create Encounter Screen will be reflected as an update of the in context encounter. To create a new encounter, we need to remove the old encounter from context. To do so, go to Subject Summary from left Navigation Panel and click on Create Encounter under the Infection Control Encounter Group
If a client has a central line, you should always create Encounter Type Central Line-Bundle-Event and fill the above UDF. However, make sure to check “inserted Outside ICU or Outside Hospital” to skip Insertion Details section. Then proceed to complete the Bundle Daily Review Section.
You should create a new UDF in the same encounter.
If the two events occurred within 14 days they will be counted as one event. However, if the 2nd event occurred after 14 days you should fill a new UDF for event and it will be calculated as 2 separate events
Create a new encounter which is end of surveillance with related reason.
You should start with the encounter "Restarting surveillance for Pt. within same admission" and the reason will be "Pt. transferred back to surveillance unit" + another encounter for unit admission If there is any device you should create new encounter and fill new UDF – Never update the previous encounters and UDFs after ending surveillance unless you need to correct any previous mistake!
